Tots-2-Teens expo Science Fair

Please review the following instructions:

1. Download the required Informed Consent Form (in PDF format). Consent can only be given by the parent or legal guardian of the student participating in the Science Fair. Complete the form and then email a scanned copy of the form to: info@totstoteensexpo.com Be sure to include "Science Fair" in the subject of your email. You may also mail your completed form to us at:

2. Download the Official Rules and Guidelines (in PDF format). If you're planning to compete in this year's Science Fair, you'll need to familiarize yourself with these rules so that there will be no suprises on the day of the Expo.

3. Download the Official Registration and Consent form (In PDF format). Please download print and complete the attached form. Complete the form and then email a scanned copy of the completed form, which much include the signature of the participating student's parent or legal guardian, to: info@totstoteensexpo.com. Be sure to include "Science Fair" in the subject of your email. You may also mail your completed form to us at:
